Test Date: Saturday 4/17 OR Monday 4/19
Filming Date: Tuesday 4/20
Rate: Testing Rate is $37.50 – Filming Rate $150/12 (minors for this age have a max 10 hours allowed on set)
Photo Double: Looking for a minor 9-12 years who appears Caucasian
Height: 5’6-5’8
Weight: 60-80 lbs
Hair Color: Blonde – Dirty Blonde (see photo reference)
Production will be requiring regular covid testing for anyone on set (provided by production) – for minors this will include the adult that accompanies them. The adult that covid tests with the minor must be the SAME adult that is on set with the minor
If your child does not have a Georgia Department of Labor number please click https://www.dol.state.ga.us/public/es/mie/minor/login and “register” them – in 24-48 hours you will receive your child’s certificate number – please store this
somewhere convenient (you’ll be asked for it later)
If your minor fits the above description and is a good match to the photo please send us the following information
1. First and last name of Minor and Parent that will be on set
2. Photos of Minor (both face and full body photos – please make sure there are no filters and no one else in the photo with your minor)
3. Date of Birth for both minor and guardian that will be on set with the minor
4. Minors Current Height and Weight
5. Minors Sizes (Shirt / Pants / Shoes)
6. Minors Georgia Department of Labor Number and Last 4 of Social (in order to permit them)
7. Parents contact information
8. Please let us know your preferred testing date Saturday or Monday
